This EuroPCR 2026 session focused on the management of advanced heart failure (AHF) using guideline-directed medical therapy (GDMT) and mechanical circulatory support devices, particularly the Impella 5.5.
Experts discussed challenges in optimizing GDMT in severe AHF patients, who often face hypotension, renal failure, hyperkalemia and complex comorbidities that limit therapy implementation.
Two detailed clinical cases were presented illustrating the use of short-term mechanical support as a bridge to transplant and as a means to enable safe GDMT initiation and titration. Hemodynamic monitoring and right heart catheterization were highlighted as crucial for assessment and management.
The session underscored the need for early referral to specialized centers for advanced therapies. Despite promising clinical experiences, speakers emphasized the lack of robust randomized evidence for these interventions and the importance of multidisciplinary care, patient selection, and individualized management strategies.
