Davide Ausiello presents a complex high-risk percutaneous coronary intervention (PCI) in an 80-year-old patient with severe ischemic heart failure and reduced ejection fraction. The patient, managing multiple comorbidities such as diabetes and end-stage renal disease, experienced repeated cardiac arrests due to ventricular arrhythmias.
Coronary angiography confirmed multivessel disease featuring severe calcifications and subocclusive stenoses. Due to hemodynamic instability and complex lesion anatomy, the PCI was performed using Impella mechanical circulatory support and rotational atherectomy to treat heavily calcified lesions.
