This session of the IHF course 2026 focuses on the integrated care of patients with aortic stenosis and heart failure with reduced ejection fraction (HFrEF), emphasizing the transition from medical therapy to percutaneous interventions like TAVI.
Two complex clinical cases highlight challenges including patient selection, procedural planning, coronary artery disease management, and the role of multidisciplinary heart teams.
The discussion addresses the importance of early guideline-directed medical therapy (including SGLT2 inhibitors and RAS blockers) before and after TAVI to improve ventricular remodeling and reduce heart failure risk.
Emerging areas include optimal timing and sequencing of PCI and TAVI, frailty assessment, and long-term management to prevent rehospitalization.
